Our longstanding commitment to the development of continuity-of-care systems for communities left behind by traditional population health management approaches has led to the ongoing development and refinement of the person-centered health neighborhood (PCHN). The PCHN is aligned with other “upstream” public health approaches sometimes referred to as “Public Health 3.0.” In the PCHN, local public health entities take the lead in identifying key community partners and linking them together in unified continuity of care chains.
Often in partnership with state public health, BHWP works with local public health and with the community providers themselves to discover deficiencies in current systems of care and to assist in designing health neighborhoods that can address them. When education, training, or consultation can fill those gaps, BHWP works with state and local public health to design tailored curricula to meet their needs.
